What is FUE Hair Transplant Surgery?

Follicular unit extraction, or FUE, is a surgical hair restoration technique in which individual hair follicles are removed from a donor area, typically the back and sides of the scalp, and transplanted into areas of thinning or baldness. Each graft contains between one and four hairs, and because the follicles are extracted one at a time rather than as a strip of tissue, the procedure leaves only small, circular healing points that are virtually invisible once healed.

The result is a donor area that looks entirely undisturbed once the scalp has settled, which makes FUE particularly suitable for patients who keep their hair short at the sides.

How FUE Works at Our Edinburgh Clinic

Every procedure at our Edinburgh clinic begins with a detailed consultation. One of our surgeons will assess your hair loss pattern, donor density and scalp laxity before recommending an approach and a graft count tailored to your specific goals.

On the day of your procedure, the treatment area is numbed using a local anaesthetic. Individual follicular units are then extracted from the donor zone using a precision micro-punch, typically around one millimetre in diameter. Once a sufficient number of grafts have been collected, tiny recipient sites are created in the thinning area and each follicle is carefully placed to match your natural hair direction and density.

Procedures range from around four to eight hours depending on the number of grafts required. Most patients find the process comfortable throughout and return home the same day.

FUE Results: What Happens After Surgery

The transplanted hairs shed within the first two to four weeks after surgery. This is entirely normal and expected. The follicles then enter a resting phase before new permanent hair growth begins, usually becoming visible from around three to four months. Full results are typically seen at the twelve-month mark, though some patients continue to see improvement up to eighteen months post-procedure.

The new hair grows, cuts and behaves exactly like your existing hair. There is no maintenance product required and no ongoing treatment needed to sustain the result.

FUE vs DHI: Which Technique is Right for You?

A question we hear frequently at our Edinburgh clinic is how FUE compares to the DHI method. Both techniques use individually extracted follicles, so neither leaves a linear scar. The core difference is in how the grafts are placed. With FUE, recipient channels are made first and grafts are placed into them separately. With DHI, a specialised implanter pen handles both steps simultaneously.

For most patients, FUE is the more flexible option, particularly where larger areas of the scalp need to be covered. DHI can offer very precise density work in more targeted zones. We discuss both options in detail at consultation so you can make a fully informed choice.

FUE vs FUT: Understanding the Difference

Some patients come to us having read about FUT surgery, which involves removing a strip of scalp tissue from the donor area. FUT can yield a high number of grafts in a single session and is sometimes appropriate for patients with very significant hair loss, but it does leave a linear scar that becomes visible if the hair is worn short.

FUE avoids this entirely. For patients who want to keep their options open in terms of hairstyle, or who are earlier in their hair loss journey and want the least invasive route, FUE is almost always the preferred approach.

Who is a Good Candidate for FUE?

FUE works well for a wide range of patients. Men experiencing male pattern hair loss at any stage are typically good candidates, as are women with diffuse thinning who have adequate donor density. We also carry out FUE for beard restoration, eyebrow transplants and hairline hair transplant refinement work. FUE is also the preferred technique for transgender hair transplant procedures, where precise single-follicle placement is essential for natural hairline feminisation or masculinisation.

Candidacy is determined primarily by donor hair density and the stability of your hair loss. A proportion of patients are better served by an alternative approach, and we will always tell you honestly if that is the case at your consultation. We do not perform procedures on patients for whom surgery is not appropriate.

FAQs

Most FUE procedures at our Edinburgh clinic take between four and eight hours. The exact duration depends on the number of grafts being transplanted. Smaller sessions of around 500 to 1,000 grafts are often completed in half a day, while larger procedures of 2,500 grafts or more will occupy a full day. You are able to rest, eat and take breaks throughout.

The procedure is carried out under local anaesthetic, so the treatment area is numb throughout. Most patients report feeling mild pressure or movement during extraction and implantation but no significant pain. Any tenderness in the days following surgery is typically managed comfortably with standard over-the-counter pain relief.

The number of grafts required varies significantly between patients and depends on the size of the area being treated, the density you want to achieve and your existing hair characteristics. A small hairline procedure may require 800 to 1,200 grafts, while a crown hair transplant or broader scalp coverage can require 2,500 to 3,500. New hair growth typically begins to emerge from around three to four months after surgery. The hair will continue to thicken and mature over the following months, with most patients seeing their full result between twelve and eighteen months post-procedure. The shed that occurs in the first few weeks after surgery is a normal part of the process and does not affect the final outcome.

FUE does not leave a linear scar. The extraction sites heal as very small, circular marks that are not visible to the naked eye once the surrounding hair has grown back. This makes FUE an excellent option for patients who wear their hair short or who may want the flexibility to do so in future.

Most patients are comfortable returning to desk-based or office work within two to three days of their procedure. More physical activity, including gym work and sport, should be avoided for around two weeks. The small scabs in the recipient area typically resolve within seven to ten days. It was an outstanding experience from start to finish. Naturally, I was unsure on which organisation to choose for the procedure, but HSHTC came across as professional and caring, as well as having a wide range of availability. There are negative reviews on all businesses, and this drives customers like myself away. However, something told me this would likely be someone with unrealistic expectations, or not reading into the procedure as a whole. I chose to ignore the negatives and read the positives. Sara the coordinator was absolutely amazing! She outlined everything and made it clear of what is expected and what is not. After a great consultation, I was passed to the next stage of meeting surgeon Dr Ditta. He presented extremely knowledgeable and passionate about what he does, so I decided to go ahead with the procedure. The surgery day was expected to be a long, but rewarding day. It started with having to shave my head (mandatory requirement for this procedure). I’d say to anyone the thought is worser than the event, and to just do it for the positives that will follow. Dr Ditta and his team were extremely caring and sensitive, especially with briefing before any painful situations pre anaesthetic. After that, I was allowed to top up when requested and leave them to do what they do best. The day as a whole was enjoyable and interesting to speak to the team to learn more about the procedure and get to know them. Lunch was provided (yay). It was lovely of them to offer, and you definitely need to eat mid procedure to keep energy levels. At the end of the day, I was given clear instruction of how to have the best success rate. It was through a video and in person. If you have questions, ask as it’s what the team is there for. The first few days of recovery can be quite awkward and uncomfortable, but I’d recommend booking 2 weeks off work to fully recover and let the grafts set. Once you get use to the sleeping situation, it’s easy and a lovely break from reality (put pillows below your knees and stack below head for pure comfort!). The aftercare products was extremely useful and easy to use. The grafts will fall out after a few weeks, it’s expected and you’re told this plenty of times!! Having a healthy lifestyle makes your hair grow quicker and thicker, so swap your diet to good healthy hair foods. When your transplanted hair grows back, it’s an amazing feeling, but only the beginning. As the months pass by, you’ll notice a massive improvement. During this process, you’ll have regular aftercare and the chance to raise any concerns. I personally didn’t have any issues and was just grateful to still be in contact and told everything is going as it should. I’m 6 months post surgery and I’m seeing the benefits hugely, and it’s not even reached maximum results yet, as this is one year. Thanks for an amazing service!

Having given some time and thought about having a Hair Transplant, I contacted Harley street hair transplant clinics. The consultant (Sara) explained everything regarding the procedure with Dr.Ditta and the technicians. From the beginning, Dr.Ditta explained the days events and how the procedures would be done beginning with taking a photo of my head and showing me where the grafts would be taken from and where they would be placed. The procedures would be done and completed in two parts, with a lunch break in between. Once completed, Dr.Ditta went through the aftercare, explaining what to do each day for 10 days. I was given capsules, shampoo, sponge, neck pillow for sleeping with and information regarding how to look after my hair. Dr. Ditta and his team were welcoming and friendly ensuring I was looked after throughout both parts of the procedure. Sara maintained regular contact to make sure recovery was going well. The results were great, patience is needed as the grafts start to grow, and then you can see the results. Extremely happy with everything, from start to finish.

Would highly recommend! Everything went very smoothly, communication was excellent throughout. I travelled from Edinburgh to Manchester for the procedure and all travel and accommodation costs were covered. Overall, my experience at the clinic was positive. Dr. Ditta was exceptional — he made me feel comfortable and at ease throughout the procedure, and his professionalism was truly reassuring. However, I was disappointed with one member of the team who, in my opinion, did not fully follow health and safety protocols. She had her hair down without a surgical cap and was using her phone while wearing gloves, then proceeded to touch my hair without changing them. This raised concerns about hygiene and cross-contamination. Her manner was also not very pleasant, which made the experience less comfortable. Although I was initially worried about the risk of infection, I felt more at ease by the end of my visit. I hope the management reviews this matter to ensure that strict hygiene and professional standards are consistently maintained. Aside from this, I was very satisfied with Dr. Ditta’s care and look forward to seeing my results.

Before the procedure I had online and face to face consultations with no obligations. These gave me all the information I needed and really put me at ease and gave me the confidence to proceed. Everyone was really knowledgeable and friendly and clearly very experienced. Booking the procedure was quick and easy and I got a date that suited me. The procedure itself went really smoothly I was able to put headphones in and listen to podcasts and was very comfortable. Afterwards I was given really detailed notes on what to do and what would happen and everything went exactly as planned. It was so reassuring knowing that advice was only a text message away and responses were always immediate. Great clinic with friendly and experienced team. I wanted my surgery done in the UK and found the clinic on Google. Process started with virtual consultations with patient advisor and then with the surgeon. FULL costs were given up front, with good finance options available. On the day I designed a new hairline with the surgeon and ended up getting about 1600 grafts iirc, which was good value for money. Clear pre- and post-op instructions given and recovery has gone as expected. Now waiting patiently for regrowth :) - Grant
