DHI and FUE are both methods of extracting and transplanting hair follicles. They share the same extraction technique, which is the defining characteristic of FUE surgery, and they differ in how extracted grafts are placed into the recipient area. Understanding the difference helps you ask the right questions at consultation, but the choice between them is ultimately a clinical one made by the surgeon based on your specific anatomy and procedure requirements.
What They Have in Common
Both DHI and FUE use the same extraction process. Follicular units are removed one at a time from the donor area using a small circular punch, typically 0.8 to 1.0mm in diameter. This is the technique known as follicular unit extraction, or FUE. The extraction leaves tiny round wounds that heal without a visible linear scar.
The distinction between DHI and standard FUE is entirely in the placement step.
How Standard FUE Placement Works
In conventional FUE, the surgeon creates the recipient sites first, before any grafts are placed. This involves making a series of small incisions in the recipient area using a fine blade or needle, at the planned angle, direction, and depth. Once all the sites are created, the extracted grafts are then inserted into each site one by one.
This approach gives the surgeon complete control over the architectural plan of the recipient area before placement begins. The angle, direction, spacing, and density of every site can be reviewed and adjusted before a single graft is committed. For complex areas such as the crown whorl pattern or a hairline with irregular natural direction changes, being able to see and assess the full site map before beginning placement has clinical advantages.
How DHI Works
In the DHI (direct hair implantation) method, a specialised implanter pen is used. The extracted graft is loaded into the tip of the implanter, which simultaneously creates the recipient site and places the graft in one motion. There is no separate incision phase.
Because the implanter both punctures and places, DHI theoretically reduces the time between extraction and implantation for each graft. Graft survival depends partly on how long extracted follicles are out of the body, and proponents of DHI argue that the simultaneous place-and-puncture reduces this interval.
DHI also means that no pre-made sites are visible during placement, which some surgeons prefer for specific areas where a fresh view of each insertion point is helpful.
Which Is Better?
Neither technique is categorically superior. The quality of the result depends primarily on the surgeon’s skill and experience, not on which of the two placement methods is used. A poor result is equally achievable with either technique; so is an excellent one.
That said, there are clinical scenarios where one technique is more appropriate than the other.
DHI tends to be preferred for:
- Procedures where the surgeon wants to place grafts without shaving the recipient area
- Smaller, targeted procedures where a single implanter can cover the recipient area without speed being a limiting factor
- Areas where the existing hair density is moderate and precise angling around existing hairs is the priority
Standard FUE tends to be preferred for:
- Larger graft counts where the efficiency of pre-made sites with a team placing into them is a practical advantage
- Complex crown whorl work where pre-planning the full site map before placement is preferred
- Hairline work where the complete design can be reviewed and refined before any grafts are committed

DHI vs FUE: Does Not Shaving Make a Difference?
One commonly cited advantage of DHI is that it can be used without shaving the recipient area. Because the implanter pen both parts and enters the skin, existing hairs in the recipient zone do not need to be shaved away before placement, as they would with a blade-created site in standard FUE.
This matters for some patients, particularly those with moderate existing density who want to maintain their current appearance throughout recovery. It is also relevant for women who want to add density to a thinning area without the telltale shave.
However, shaving the donor area is still required in both techniques. The ability to avoid shaving the recipient area with DHI does not mean the procedure can be completed without any shaving.
Recovery: Is There a Difference?
Recovery after DHI and FUE follows essentially the same timeline. Both are day procedures under local anaesthetic. There is no meaningful evidence that one technique produces faster healing or reduced redness compared to the other. Post-procedure care is the same regardless of which placement method was used.
